Your first month on a GLP-1: side effects, dose steps and when to call

In your first month on semaglutide or tirzepatide, you are on the lowest starting dose, and the most common things you will notice are a smaller appetite, nausea, and changes in your bowels such as constipation or diarrhea. These effects are usually mild to moderate and tend to ease as your body adjusts, which is exactly why both drugs start low and step up every four weeks. Call your prescriber promptly for severe or persistent abdominal pain, pain in your upper right abdomen, signs of dehydration, or low blood sugar if you also take insulin or a sulfonylurea.

The step by step guides cover how to get a prescription. This post covers what comes after the first shot: the questions people actually ask in weeks 1 to 4.

How a medical GLP-1 start works

Who are these medications approved for? Wegovy (semaglutide) and Zepbound (tirzepatide) are FDA approved for chronic weight management in adults with a BMI of 30 or higher, or 27 or higher with at least one weight-related condition such as high blood pressure, type 2 diabetes or high cholesterol. Both are meant to be used alongside a reduced calorie diet and more physical activity, according to the FDA prescribing information for each drug on DailyMed.

What should happen before your first dose? A real medical evaluation, not a checkbox form. Your prescriber should review your history, medications and labs, and screen for reasons you should not take these drugs. Both labels list a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 as contraindications.

How does it work at BioCure? You complete a five minute online intake, then a video consultation with a physician ($199, credited back if you start a membership). A blood panel is required before semaglutide or tirzepatide, done with an in-home phlebotomist or at a lab, with results in 7 to 14 days. Your physician walks you through the results and your protocol 1:1, and medication ships to your door, typically 5 to 10 business days after the consultation and testing. Our medical weight loss program includes 24/7 support and regular check-ins with protocol adjustments, which matters most in exactly the weeks this post covers.

Common early effects and how dose steps affect them

What side effects are normal in the first weeks? Both labels list nausea, diarrhea, vomiting, constipation and abdominal pain among the most common side effects. A smaller appetite and feeling full sooner are the intended effects, not side effects. These drugs slow how quickly your stomach empties and act on appetite signals, so eating your usual portion can suddenly feel like too much.

Why do side effects flare after a dose increase? The labels report that stomach and bowel side effects happen most often during dose escalation. Each step up is a new adjustment. Stanford Medicine's GLP-1s 101 guide makes the same point: side effects are mostly gastrointestinal, and the slow ramp up is how they are kept manageable.

What does the standard dose schedule look like?

Weeks Wegovy (semaglutide), once weekly Zepbound (tirzepatide), once weekly
1 to 4 0.25 mg (starting dose) 2.5 mg (starting dose)
5 to 8 0.5 mg 5 mg
9 to 12 1 mg 7.5 mg
13 to 16 1.7 mg 10 mg
17 and beyond 2.4 mg maintenance (1.7 mg if 2.4 mg is not tolerated) Can continue rising by 2.5 mg every 4 weeks; maintenance is 5, 10 or 15 mg

This is the schedule in the FDA labels. The starting doses are designed to let your body adjust, not to be your long term treatment dose, so modest change on the scale in month one is expected. If a step is hard to tolerate, the Wegovy label allows your prescriber to delay the next increase, and in practice a prescriber can hold you at any step longer. If your prescription is a compounded medication, follow your prescriber's schedule; the FDA labels remain the best reference for safety warnings.

Constipation is the effect people most often underestimate. Our guide to preventing constipation on weight loss medications covers it in detail. If you feel drained or weak, read why people feel weak on GLP-1s and how to fix it.

Warning signs that mean you should call

Most early effects are uncomfortable rather than dangerous. The labels describe a short list of symptoms that are different, and you should know them before your first shot.

Symptom What it could signal What to do
Severe abdominal pain that does not go away, sometimes spreading to your back, with or without vomiting Pancreatitis Stop your dose and seek care right away
Pain in the upper right abdomen, fever, yellowing skin or eyes, clay colored stools Gallbladder problems such as gallstones or inflammation Call your prescriber promptly
Vomiting or diarrhea that keeps you from holding fluids down, dizziness, very little or dark urine Dehydration, which the labels link to kidney injury Call the same day
Shakiness, sweating, confusion, fast heartbeat, intense hunger while also taking insulin or a sulfonylurea Low blood sugar Treat with fast acting sugar, then call; your other medication may need adjusting
Swelling of the face, lips, tongue or throat, trouble breathing, severe rash Serious allergic reaction Get emergency care
A lump or swelling in your neck, hoarseness, trouble swallowing Possible thyroid tumor (a boxed warning on both labels) Tell your prescriber

One more item from the labels: if you have surgery or a procedure with anesthesia or deep sedation, tell the team you take a GLP-1, because slowed stomach emptying raises the risk of inhaling stomach contents.

Tirzepatide and the birth control pill

Does Zepbound affect birth control pills? It can. The Zepbound label says tirzepatide may make oral hormonal contraceptives less effective, because slower stomach emptying can reduce how much of the pill you absorb. The label advises switching to a non-oral method, or adding a barrier method such as condoms, for 4 weeks after you start Zepbound and for 4 weeks after each dose increase.

That means the backup window opens again at every step in the table above. The Wegovy label does not carry the same backup contraception instruction, though it notes semaglutide slows stomach emptying, which can affect how oral medicines are absorbed. Neither drug should be used during pregnancy for weight loss, so tell your prescriber if you are pregnant or planning to be.

Missed doses and restarting

What if I miss my Wegovy dose? Per the label, take it as soon as you remember if your next scheduled dose is more than 2 days away. If it is less than 2 days away, skip it and take the next dose on your usual day. If you miss more than 2 doses in a row, the label lets your prescriber either resume your schedule or restart the dose escalation, which can reduce stomach side effects on restart.

What if I miss my Zepbound dose? Take it within 4 days of the missed dose. If more than 4 days have passed, skip it and resume on your regular day. The label does not spell out a restart schedule after a longer gap, so check with your prescriber rather than jumping straight back to your last dose.

Can I change my injection day? Yes. Wegovy allows it if at least 2 days separate the two doses, and Zepbound if at least 3 days separate them.

The FDA's removal of the suicidal ideation warning

The weight-management versions of GLP-1 medicines, including Wegovy, Zepbound and Saxenda, carried a warning about suicidal behavior and ideation, a warning historically placed on weight-loss drugs as a class. In a drug safety communication, the FDA requested that manufacturers remove it after its review of the available data did not find evidence that these medicines cause suicidal thoughts or actions.

What this means for you: a history of depression or anxiety does not automatically rule you out. It still belongs in your intake, and any new or worsening mood change during treatment is worth reporting, as it would be with any new medication.

Eating and hydration habits that reduce side effects

How you eat in month one shapes how you feel more than almost anything else.

Frequently asked questions

Is it normal to lose very little weight in the first month on semaglutide or tirzepatide?

Yes. The first month uses the starting dose, which the labels describe as a dose to help your body adjust rather than a maintenance dose. Most of the effect builds as your dose steps up over the following months.

How long does nausea last when starting a GLP-1?

For most people, nausea is mild to moderate and eases as the body adjusts. The labels report stomach side effects happen most often during dose escalation, so it may return briefly after each increase. Nausea with severe abdominal pain or vomiting that stops you keeping fluids down needs a call to your prescriber.

Do I need backup birth control on Zepbound?

If you take birth control pills, the Zepbound label advises switching to a non-oral method or adding a barrier method for 4 weeks after starting and 4 weeks after each dose increase. Tirzepatide can reduce how well the pill is absorbed.

What happens if I miss my Wegovy or Zepbound shot?

For Wegovy, take it if your next dose is more than 2 days away; otherwise skip it. For Zepbound, take it within 4 days; otherwise skip it. After several missed doses, ask your prescriber whether to resume or restart the dose steps.

Did the FDA remove the suicide warning from Wegovy and Zepbound?

The FDA requested removal of the suicidal behavior and ideation warning from GLP-1 weight-management labels after its review did not find evidence these drugs cause suicidal thoughts or actions. You should still report any new or worsening mood changes to your clinician.

Can I start a GLP-1 weight loss program through telehealth?
