Is your hormone plan really personalized? 8 signs it's a template
A personalized hormone optimization plan is built on your own blood work, your full medical history, and a written schedule for retesting and adjusting your dose. If your plan started from a questionnaire alone, gives you the same dose as everyone else, or has no follow-up labs on the calendar, it is a template with your name on it. Use the eight signs below, grounded in Endocrine Society and FDA guidance, to check any plan you are on or are being offered.
"Personalized" has become a marketing word. Local hormone practices and national telehealth brands alike say their plans are tailored. The Endocrine Society has specifically cautioned that compounded hormones are often sold as customized when that customization is not backed by reliable testing or regulation. So the useful question is not whether a provider uses the word. It is whether the plan shows the work.
The 8-point checklist at a glance
| # | Sign of a personalized plan | Red flag of a template | Question to ask your provider |
|---|---|---|---|
| 1 | Diagnosis built on your labs | Treatment offered after a symptom quiz alone | Which of my lab results support this treatment? |
| 2 | Dose adjusted from follow-up labs | Same starting dose for everyone, never revisited | When do we recheck, and what would change my dose? |
| 3 | Fertility, heart, prostate or breast history reviewed | No questions about family plans or cancer history | How does my history change your recommendation? |
| 4 | Every medication explained | Add-ons bundled in with no stated purpose | Why is each item in my protocol? |
| 5 | Sleep, training and weight addressed | Prescription is the only lever | What should I change outside of medication? |
| 6 | Written monitoring schedule | "Call us if something feels off" | Can I see my monitoring plan in writing? |
| 7 | FDA-approved options offered where suitable | Only compounded products, framed as superior | Is this product FDA-approved or compounded, and why? |
| 8 | Direct access to the prescriber | You only ever talk to sales or support staff | Who writes my prescription, and how do I reach them? |
If you count several red flags, you are not on a personalized plan. You are on a protocol that happens to include you.
Signs 1 and 2: Your labs drive the diagnosis and the dose
1. Built on your labs, not a questionnaire alone
Low energy, poor sleep, low libido and stubborn weight gain overlap with thyroid problems, sleep apnea, anemia, depression and medication side effects. A questionnaire can start the conversation. It cannot make the diagnosis.
The Endocrine Society's 2018 testosterone therapy guideline recommends diagnosing low testosterone only in men who have both symptoms and consistently low testosterone, confirmed by repeat morning fasting blood tests. A plan that prescribes testosterone from a single result, or from no result at all, skips that step.
Breadth matters too. A personalized plan looks beyond one hormone to thyroid, metabolic, lipid, blood count and organ markers, because those results shape whether treatment is safe and which treatment fits. That is why BioCure starts with diagnostics: a Comprehensive Male or Female panel covering 90+ biomarkers, drawn by an in-home phlebotomist or at a lab, with results in 7 to 14 days.
2. Dose chosen, then adjusted from follow-up labs
A starting dose is an informed estimate. Real personalization happens afterward, when your clinician retests, sees how your body actually responded, and adjusts. The Endocrine Society guideline describes treating toward the mid-normal range rather than chasing the highest number possible, and it calls for checking hematocrit because testosterone can raise red blood cell counts.
If your dose has never changed and nobody has looked at a follow-up result, ask why. Two people on the same dose can end up with very different levels.
Signs 3 and 4: Your history shapes the plan, and every medication has a reason
3. Considers fertility, cardiovascular and prostate or breast history
This is where templates do the most harm. Testosterone therapy suppresses the body's own sperm production, and the Endocrine Society guideline recommends against it in men who plan to have children in the near term. The same guideline lists situations where testosterone is not advised, including breast or prostate cancer, untreated severe obstructive sleep apnea, uncontrolled heart failure, a recent heart attack or stroke, and an elevated hematocrit.
For women, a history of breast cancer, blood clots or cardiovascular disease shapes whether estrogen is appropriate and which form makes sense. A provider who never asks about these things is not personalizing anything.
Fertility goals should change the options on the table. For men who want to support their own production, BioCure's bioRestore tier uses medications that raise the body's own testosterone without direct hormone replacement, while bioTest is full replacement with ongoing hormone monitoring. Which one fits depends on your labs, history and goals. More detail is on our testosterone therapy page.
4. Explains why each medication is included
Template plans bundle. You get the hormone plus a set of add-ons that every patient receives regardless of their labs. In a personalized plan, each item maps to a specific finding or goal, and your clinician can tell you in one sentence why it is there and what would make them remove it.
Signs 5 and 6: Lifestyle levers and a written monitoring schedule
5. Includes lifestyle levers: sleep, training, weight
Hormones do not operate apart from the rest of your life. Body weight, sleep quality, alcohol and training all influence hormone levels and how you feel on treatment. Untreated severe sleep apnea is one of the conditions the Endocrine Society guideline flags before starting testosterone, which tells you how closely sleep and hormones are linked.
A plan that hands you a prescription and nothing else is leaving results on the table. If you are younger and weighing whether you need hormones at all, our guide to hormone optimization in your 30s walks through when lifestyle changes come first.
6. Has a written monitoring schedule
You should be able to see, in writing, what will be tested, when, and what result would trigger a change. The Endocrine Society guideline sets out structured monitoring after testosterone is started and at regular intervals afterward, including testosterone levels, hematocrit and, for appropriate men after a shared discussion, prostate screening.
For women, the Endocrine Society's position statement notes that salivary hormone testing, often used to justify custom compounded doses, is not a reliable way to guide dosing. A monitoring plan built on validated blood tests and symptom review is a stronger sign of real personalization.
Signs 7 and 8: FDA-approved options and real access to your prescriber
7. Offers FDA-approved options where suitable
The Endocrine Society's position statement on compounded bioidentical hormones is direct: compounded hormones are not FDA approved, and they are not held to the same standards of testing for purity, potency and safety as approved products. The Society also points out that FDA-approved bioidentical options exist, meaning hormones chemically identical to the ones your body makes.
Compounding has legitimate uses, such as an allergy to an ingredient in a commercial product or a dose that is not commercially available. The red flag is a provider who offers only compounded products and frames them as inherently more personalized. Ask whether each product is FDA approved or compounded, and why that choice fits you.
The FDA's testosterone information page is worth reading too. It explains that approved testosterone products are indicated for men with low testosterone caused by certain medical conditions, and it summarizes the agency's safety communications. A good provider will discuss where your situation fits.
8. Gives you access to the prescriber
You should know who your licensed prescriber is and how to reach them. If every interaction runs through a sales coordinator or a chat window, your plan is unlikely to be adjusted thoughtfully when something changes.
At BioCure, a physician reviews your results with you in a 1:1 custom protocol review, by video or in person at our Fort Lauderdale area clinic. Ongoing follow-up and support are included, and the team replies to new inquiries within 24 hours.
How to choose a safe hormone therapy provider in Florida
Whether you are searching in Parkland, Tampa or anywhere else in Florida, you have three broad options, and the checklist applies to all of them.
| Provider type | Best fit for | How pricing usually works |
|---|---|---|
| Board-certified endocrinologist | Complex endocrine disease, such as pituitary or adrenal disorders | Usually billed to insurance, with labs and medication billed separately |
| Primary care physician | Straightforward cases inside your existing care | Usually billed to insurance |
| Telehealth or optimization clinic | Labs-first optimization with ongoing monitoring and convenience | Usually cash pay, often as a membership or bundle; check what labs, consults and medication include |
BioCure Health is a physician-led telehealth longevity clinic with a physical clinic in the Fort Lauderdale area, licensed to treat patients in 47 states, with availability in your state confirmed at intake. The physician video consultation is $199 and can be credited back with a membership. Medications are prepared through licensed compounding pharmacy partners or standard pharmacies as appropriate and ship to your door, typically 5 to 10 business days after your consultation and testing. BioCure does not accept insurance directly, though many patients use HSA or FSA funds with the invoices we provide.
We are a strong fit if you want a plan built on comprehensive labs and adjusted over time. If you need insurance coverage, or you have a complex endocrine condition, a board-certified endocrinologist is the right first call.
Frequently asked questions
Are compounded bioidentical hormones more personalized than FDA-approved hormones?
Not necessarily. The Endocrine Society cautions that compounded hormones are not FDA approved and are not held to the same testing standards for purity, potency and safety. FDA-approved bioidentical options exist, and personalization comes from your labs, history and dose adjustments, not from the pharmacy that mixes the product.
How often should my hormone levels be checked on testosterone therapy?
The Endocrine Society guideline calls for structured monitoring after starting testosterone and at regular intervals afterward, including testosterone levels and hematocrit, plus prostate screening for appropriate men. Your provider should give you this schedule in writing. If no follow-up labs are planned, treat that as a red flag.
Can I get a personalized hormone plan online without an in-person visit?
Yes, if the telehealth provider orders real blood work, reviews your full history and gives you access to the prescriber. A plan based on a questionnaire alone is not personalized. BioCure patients can do every appointment by video or in person, with labs drawn at home or at a lab.
Should I see an endocrinologist or a hormone clinic in Florida?
See a board-certified endocrinologist if you have complex endocrine disease or need insurance billing. A physician-led hormone or longevity clinic can suit healthy adults who want labs-first optimization and close monitoring. Either way, run the provider through the same eight-point checklist.
Does insurance cover a personalized hormone optimization plan?
Coverage varies widely, and many optimization clinics are cash pay. BioCure does not accept insurance directly, but many patients use HSA or FSA funds, and we provide invoices and documentation for reimbursement.
A hormone plan should read like it was written for one person, because it was. If yours cannot pass this checklist, you deserve a plan that is monitored, explained and adjusted around your body, not a template.
